The TANDEMplus mobile crisis team works closely with the person in crisis and his or her environment. The approach is primarily intended to alleviate the crisis and to guide the client in building a care network. After 7 years of operation, we conclude that the continuity of care for people with mental disorders remains problematic. The few data we were able to analyze underline the importance of active collaboration between the mobile crisis team, the mobile long-term care teams and the ambulatory care services.
